docs: Document issue lifecycle and stale prevention methods

Add documentation for issue lifecycle management
including how to prevent issues from being marked as stale using
assignees, exempt labels (pinned, security, good-first-issue,
help-wanted), regular activity, and converting to discussions.

Also add 'assignees' to spelling wordlist.
